New Patient Information

In preparation for your child’s evaluation: 

  • Obtain a prescription from your pediatrician for evaluation and treatment sessions.

  • Complete the secure, online intake form below at least a day in advance so we can verify your insurance and our therapists can prepare

  • Bring with you any evaluations or documents that you feel may be important for the Therapist to review.

Referral Process and Insurance Policies

Patients maybe self-referred, referred by physicians, or by other health care professionals. We are an in-network provider for many insurance companies in the state of Pennsylvania. Pre- authorization and filing of insurance are services provided for patients by our clinic.

  • Highmark BC/BS (and all BC/BS plans)

  • United Healthcare

  • Cigna

  • Aetna

  • Medicaid Insurances Accepted - Medicaid Access and Highmark Wholecare (ending 9/30/2024)

  • UPMC for You - WE ARE NOT IN NETWORK AND DO NOT ACCEPT UPMC

New Patient Forms

Once you make an appointment, you will need to complete our INTAKE PACKET. Please note that you will need to upload a picture of the front and back of your insurance card. You can access the form by clicking here. Also note that we require a $25 deposit to secure your evaluation appointment and all patients must agree to keep a credit card on file with our billing agent. There are no exceptions to this policy.
